A transferrin test directly measures its level in the blood. Alternatively, transferrin may be measured indirectly (or converted by calculation) so that its level is expressed as the amount of iron it is capable of binding. This is called the total iron binding capacity (TIBC).

TIBC is a measurement of how well your blood attaches itself to iron to move it around your body. Ferritin is a protein in the blood that contains iron and helps you understand how much iron your body is storing.

Another measurement, called transferrin saturation, checks how many places on your transferrin that can hold iron are actually doing so. Normal values are 15% to 50%. In severe cases of iron-deficiency and anemia, this number may fall below 10%. Many other health conditions can cause high or low levels of transferrin.

If you have iron deficiency anaemia (a lack of iron in your blood), your iron level will be low but your TIBC will be high. If you have too much iron (for example, if you have a condition like haemochromatosis), your iron level will be high but your TIBC will be low or normal.

High TIBC means that you have low levels of iron. When there isn't much iron to attach to, you'll have a lot of free transferrin in your blood. Low iron levels can lead to iron deficiency anemia, a condition in which your body doesn't have enough iron to make healthy red blood cells.

Preparation. During your fast, you may drink water, but it must not contain additives e.g. avoid tea, coffee or cordials. You should avoid alcohol for 24 hours or 1 day prior to the test, if possible. Unless your doctor advises otherwise, you should continue taking any current medications.

Do not eat or drink (other than water) for 12 hours before the test. You will also need to avoid iron supplements for 12 to 24 hours before your test. Your doctor will give you specific instructions. This test is often done first thing in the morning.
